What lipedema is
Lipedema is an abnormal and painful distribution of subcutaneous fat that especially affects legs, hips and arms. Its key characteristic is disproportion: slim trunk vs. voluminous limbs, with feet and hands respected (cuff sign).
It is a chronic disease recognised by the WHO. It usually appears at hormonal changes: puberty, pregnancy, menopause.
Lipedema vs obesity vs cellulite
- Lipedema: disproportionate distribution, pain on pressure, spontaneous bruising, does NOT respond to diet. Feet respected.
- Obesity: generalised fat increase, responds to diet and exercise, no pain.
- Cellulite: aesthetic alteration of subcutaneous tissue, no pain or progression, no bruising.
There are patients with lipedema + obesity simultaneously. Obesity can be treated with diet and exercise; the lipedema component cannot.
The 5 types according to location
- Type 1: hips and buttocks. The typical "pear" shape.
- Type 2: hips + thighs to knees.
- Type 3: hips + thighs + calves to ankles.
- Type 4: arms (frequently associated with previous types).
- Type 5: calves only.
The type determines the surgical areas to be treated and the number of sessions needed.
The 4 stages of progression
The transition between stages can take decades or be rapid. Hormonal triggers (pregnancy, menopause) usually accelerate it. That is why surgery in Stage I–II has a preventive role in addition to therapeutic.
When and how to operate
Lipedema surgery is not aesthetic liposuction. WAL (Water-Assisted Lipoplasty) or VASER techniques are used with thin cannulas and trajectories parallel to lymphatic vessels to preserve the lymphatic system while removing diseased fat.
Surgical indications:
- Confirmed clinical diagnosis of lipedema
- Significant symptoms (pain, functional limitation, deterioration of quality of life)
- Lack of response to conservative treatment (lymphatic drainage, compression garments)
- Stable weight for 6 months
- General health that allows general anaesthesia
