What is lipedema?

Lipedema is a chronic disease of the subcutaneous fat that mainly affects women and is characterised by an abnormal, painful distribution of fatty tissue, particularly in the legs, hips and arms. It is neither obesity nor cellulite: it is a hormonally driven medical condition that does not respond to diet or exercise.

Left untreated, lipedema progresses over the years, causing pain, spontaneous bruising and functional limitation, and in its advanced stages it can be associated with secondary lymphedema.

Symptoms that point to the diagnosis

  • Clear disproportion between a slim torso and voluminous legs or hips
  • Pain on pressure, a feeling of heaviness and cramps
  • Bruising from minimal knocks
  • Ankles spared (cuff sign)
  • No response to strict diets or training
  • Family history of similar presentations

Stages of lipedema

Stage 1

Smooth skin, increased and soft subcutaneous fat. Mild symptoms.

Stage 2

Irregular surface, palpable nodules, more constant pain.

Stage 3

Evident deformity with hanging fat lobules and functional limitation.

Stage 4

Lipedema associated with lymphedema (lipo-lymphedema). Multidisciplinary management.

Lipedema surgery is not aesthetic liposuction. We use WAL (Water-Assisted Lipoplasty) or VASER techniques with fine cannulas and passes parallel to the lymphatic vessels to preserve the lymphatic system while removing the diseased fat. It is usually planned in several sessions according to the stage and the area treated.

Frequently asked questions

Does surgery cure lipedema?
It is not curative, but it is the most effective treatment to halt progression, reduce pain and improve contour. The diseased fat removed does not form again in that area, although lifestyle habits and medical management remain important.
How many sessions will I need?
It depends on the stage and the number of affected areas. Usually between 1 and 4 sessions, spaced 8–12 weeks apart. It is planned individually in consultation.
Is it covered by the Spanish public health system?
In Spain public coverage of lipedema is very limited and is usually restricted to advanced stages with associated lymphedema. Private consultation gives access to treatment in early and intermediate stages.
How much does lipedema surgery cost in Barcelona?

Do I have to lose weight before surgery?
Losing weight is not essential because lipedema fat does not respond to diet. We do look for a stable weight and a healthy lifestyle before surgery.
